Students who are placed on Satisfactory Academic Progress (SAP) Probation or Termination may request Reinstatement under the standard College Policy. If you have met the requirements for Reinstatement, please indicate the basis for SAP review and submit this form to the Financial Aid Office for evaluation.

Students who have received a grade change may request reinstatement of good standing, by finishing an incomplete class with a 2.00 GPA or better, or through a grade change that raises the GPA to a 2.00 or above that successfully meets the SAP requirements at Clark College.
Students who have been terminated based on SAP requirements may have future financial aid eligibility restored if a sufficient number of degree or certificate relevant credits are earned at a 2.00 GPA or better through funding other than financial aid. The SAP Termination Notice (No Further Funding Letter) identifies the number of credits that must be earned to accomplish reinstatement. Grades acceptable for Reinstatement include A, B, C, and S - though a student may not be reinstated with only S grade credits - earned in courses applicable to the program.
