Instructions

Please complete this form and enter all required information. To complete the identity verification, you must upload:

Picture of the front of government issued identification card showing your full legal name and date of birth.
Picture (selfie) of you holding your identification card next to your face to confirm your identity.
The pictures can be submitted in one document/file, or as two separate files.

You can use the following forms of identification:

DMV Issued Driver's License or Non-Drivers Identity Card
US and Foreign issued Passport
United States Passport Card
US Military & US Military Dependent's ID Card
Tribal Card
If you have any questions or concerns regarding acceptable forms of identification cards, please contact Enrollment Services via email (enroll@clark.edu), by phone (360-992-2107), or in person by visiting Gaiser Hall 128.

If you do not wish to use this form to complete your identity verification, you will need to come to Enrollment Services in-person. Please contact Enrollment Services as soon as possible to verify your identity.

If you are receiving an invalid student ID error, please contact enroll@clark.edu with your full name and ctcLink Student ID number.
