Clark College provides Basic Term Life and Accidental Death & Dismemberment Insurance for benefit-eligible employees at no cost to the employee. Basic coverage is provided in the amounts of:
$25,000 Basic Life Insurance for death from any cause
$5,000 Basic Accidental Death & Dismemberment Insurance

Employees may purchase additional term life supplemental insurance coverage for themselves, basic and supplemental coverage for their spouse, and basic coverage for each eligible dependent child. See page 4 of the Life Insurance Booklet for a description of eligible dependents. Newly eligible employees have 60 days from their start date to apply for certain amounts of life insurance coverage without providing medical history. After the 60-day grace period ends, there is no open enrollment period to make changes in this plan without approval from ING. If you wish to increase coverage after your grace period ends, medical history will be required (Evidence of Insurability Form) and approval will be determined by ING/Reliastar Life Insurance.
Employees may also purchase additional Accidental Death & Dismemberment Insurance without medical history and approval by ING/Reliastar.
