Students who are under 18 and/or are enrolled in high school are required by Labor and Industries to have the following form signed by parent/guardian and/or high school counselor. Once this form is completed and signed please return it to the Financial Aid Office. (Create link).

If you are over 18, please go to Admissions with a current state issued ID to have your date of birth corrected in our systems.
