Service Learning Forms and Procedures
Are you ready to start your service-learning?
Here are the next steps to get you on your way:
Step 1: Find a service-learning site
- Your instructor may have a pre-approved list of service-learning
sites.
- If this is the case, review the list and identify a site the best meets your learning objectives,
schedule, and interests.
- Contact the site, and inquire about service-learning opportunities.
- Clearly state that you are a Clark College student completing a service-learning requirement.
Be specific with the site about the number of hours you are required to complete.

Volunteer Data Base to find a site that
best meet your learning objectives, schedule and interests.
Step 2: Fill out your Service-Learning Site Agreement & Liability Form
Return the completed Service-Learning Site Agreement & Liability Form to the Service-Learning Program,
Gaiser Hall 204c.
Step 3: Print out the Time Sheet and Agency Evaluation
With these forms, keep track of your hours and at the end of your service complete the Student Evaluation
and have your site supervisor complete the Site Evaluation. Your Time Log and Student Evaluation must be
submitted to your instructor by the deadline.
Step 4: Participate in your service-learning experience and reflection
- Coordinate with your site to define your project, work-times, and responsibilities.
- Attend any orientations or interviews required by your site.
- Ask questions if you are unsure about your responsibilities and anything else you have concerns about
- Respect the confidentiality of people, site and employees you serve.
- Be open to new experiences, cultures, and ways of thinking.
***Please remember you are representing Clark College in the community.
- Complete all reflection activities as assigned by your instructor.
Step 5: Turn in your Time Sheet and Site Evaluation to your instructor.
