Online Mental Health Screening

Brief self assessments for depression, bi-polar disorder, generalized anxiety disorder and post traumatic stress disorder are available.
Since people can suffer from more than one of these disorders at the same time, or one can cause symptoms that seem like the other, you may want to take all the screenings. Each screening will take approximately 5 minutes to complete.

Your score is informational, not diagnostic.
It indicates whether your responses are consistent with depression, bipolar disorder, generalized anxiety disorder or post traumatic stress disorder and where to call for help. It is not intended to be used as a substitute for a visit with a health care professional. Clark College has counselors available to meet with currently enrolled students to discuss the results of your online screening at your request. Counselors are available in the Counseling Center in room 144 “T” building and in Health Services at 124 Health Sciences Building. If you are not a currently enrolled student you may want to talk to a counselor or health care provider in the community at Columbia River Mental Health 360-993-3003, the Clark County Crisis Line at 360- 696-9560 or Multnomah County Crisis Line at 503-988-4888. Or you may call 211 for community resources.

When you click on the link below, you will leave Clark College’s web page.
The College does not manage or control web sites not owned by the college.

A core value and objective of this screening is to preserve participant’s anonymity. Clark College cannot identify you through your responses.
You will not be asked for identifying information. The only information that can be reported to the college is general information regarding the number of screenings completed, the results of the screenings, and general demographic information. If you prefer to not provide that information, each demographic question allows a “no response” answer. Answering “no response” will have no impact on the results of the test.

If you have any questions or concerns about the privacy and security of the online screening tool, please review the privacy policy prior to beginning the screening.
