REQUIREMENTS FOR LICENSURE
After completing an accredited dental hygiene program, graduates apply for
licensure in the state(s) of their choice. The appli-cation for R.D.H. licensure
in the state of Washington includes the following personal data questions,
which are to be answered "yes" or "no." If any of the following questions
are answered "yes," full details must be provided with the application for
licensure. Washington reserves the right to deny a license when appropriate.
It is important that you are aware of these questions prior to starting
the program.
- Can you specifically list licenses granted as temporary, reciprocity, exemption or similar with type, date, and grantor, as well as state
whether license is current or not?
- Have you ever been called before any state board for interrogation concerning unethical conduct or any violation of the laws and/or rules
pertaining to the profession for which you are applying?
- Have you ever been convicted of a felony or misdemeanor other than traffic violations?
- Have you ever been convicted of a violation of the Controlled Substance Act or any narcotic law?
- Have you ever had a license to practice revoked or suspended?
- Have you ever been addicted to or treated for addiction to narcotic drugs?
- Have you ever received psychiatric treatments or received treatment for a mental illness?
- Have you ever engaged in the excessive use of alcohol or received treatment for alcoholism?
- Have you ever been denied the right to take a licensing examination in any state?
- After successfully completing a licensing exam, have you ever been denied alicense in any state?
